Job Description

For more than 30 years, the London InterCommunity Health Centre has provided health and social services to those who experience barriers to care including poverty, homelessness, language or culture, and complex and/or chronic health conditions including mental health and addictions. Today, with four busy clinics in three of London’s most under-served regions, the organization is viewed as a community leader in newcomer health, care of vulnerable women, issues of health equity, client-led service development and the advancement of partnerships that improve access to vital services for the community’s most vulnerable.

About the Position
 
Working to a full scope of practice, the Registered Nurse (RN) provides front line client care, outreach, testing and health promotion, to clients living with, or are at risk of contracting infectious diseases including Hepatitis C, HIV/AIDS and other blood-borne pathogens. The RN will work collaboratively with community partners such as the Regional HIV/AIDS Connection and the Infectious Diseases Care Program at St. Joseph’s Hospital.

The RN works as part of a multidisciplinary team  of Primary Care Physician, Infectious Disease Specialist, Social Workers, Outreach Workers to provide street- level and agency outreach, as well as onsite nursing care to people living with Hepatitis C & HIV/AIDS. These populations face barriers to accessing HIV/AIDS/HCV treatment and are significantly marginalized by poverty, drug addiction, and mental health and housing instability. The RN proactively locates clients and meets them at a street level, engages clients for stability (i.e. housing, income support, food support), provides them with education regarding their diagnosis and treatment options, engages them in treatment, monitors their viral loads, and supports the daily dispensation of their medicines.  The RN also coordinates monthly treatment clinics in partnership with the Infectious Diseases Care Program.

The RN also functions and participates in the development, implementation, monitoring and evaluation of programs, services and quality improvement initiatives for the communities served. The successful candidate will act with professionalism and courtesy toward clients, the general public and colleagues, and will demonstrate knowledge and valuing of the clients’ life situation.
